Sydenham Together Social Prescribing Project.
Croft Medical Centre and the Sydni Centre have successfully been running an exciting new Social Prescribing project since January 2016.
Social Prescribing provides alternative solutions for improving the health and well-being of communities. Placing greater emphasis on preventative interventions and reduce costly interventions in specialist care. It links patients in primary care and their carers with non-medical sources of support within the community.
Our aspirations are to develop community resilience for Sydenham North and West and address health and social inequalities. At the same time we will expand the local knowledge and range offered by our GP’s to add value for every patient that will access GP services in our local area; an aspiration where all partners (Public Health, the Local Authority and the Voluntary Sector) are working in a cohesive way to support the community and to help improve the health and wellbeing of the residents of Sydenham.
