To help us offer the appropriate advice, please fill out the online form before coming to see the nurse. Normally you need to complete this questionnaire prior to your appointment. Please note that you need to book your appointment with the travel nurse at least 6 weeks prior to travel, you can do this by telephoning reception on 01926 421153 after 10am in the morning.

We are a registered Yellow Fever Centre and provide a full range of travel vaccination services as well as a travel pack.
If you require any vaccinations relating to foreign travel you need to make an appointment at least a month in advance of your departure date with the practice nurse to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required. Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because some travel vaccines are not included in the services provided by the NHS and therefore is a chargeable service.
On your returnIf you ill on your return and need to see the doctor, don't forget to mention that you have recently travelled abroad.
If you receive medical treatment whilst abroad, make sure you tell the surgery.
If you donate blood, tell the transfusion staff which countries you have visited.
