Named Allocated GP

From April 2015 all practices are required to provide their patients with a named GP who will have overall responsibility for the care and support that the surgery provides to them.

The allocated named GP will have overall responsibility for the care and support that our surgery provides to you. This does not prevent you from seeing any GP in the practice as you currently do. 

You do not need to take any further action. If you wish to be told the name of your named GP, please ask the receptionists when you are next in the surgery. 

If you are unhappy with the named doctor allocated to you and have a genuine concern that this doctor will not be best placed to undertake the responsibilities assigned; then you can ask to be reallocated to another doctor.

Patient Registration

registrationThe practice only accepts new patients whose permanent residence is an address within our catchment area

Prospective patients need to complete a New Patient Questionnaire and a GMS 1 form.

These are provided at the surgery and can be completed either in the surgery or at home.

We will ask you supply a suitable form of Photo ID e.g. Passport, Driving Licence, proof or your address, and your current NHS number.

We also accept Online Pre - Registrations if you hold an existing NHS Number.  (Please see below). Patients will be asked to sign the GMS1 at the time of there first appointment.

An appointment with a Nurse may be necessary if you are on repeat medication.

If you move outside our catchment area you may need to register with the local surgery.
